• Commitment Form, Student Conduct Agreement, Medical Form  Haga clic aquí para imprimir los formularios de firma en español. The Normal Marching Band uses signNow to collect these forms.  When you click the link above, you will be prompted to rename these forms as the student's last and first names, for example:  Carter Paul.  You will also be prompted to enter an email address for the parent or guardian who is completing the forms as well as the student who is signing up for the NMB.  Links will be emailed to each of the addresses that you provide.  Both the student and guardian should follow the link from their email and complete the forms.  To ensure that the parent and student information end up on the same version of the document, please only click the link above once per NMB member.  These forms are due for winds and percussion on March 18 and for color guard at the clinics. ​
